Progress and challenges in clinical research of new drugs for osteoarthritis

摘要

骨关节炎(osteoarthritis,OA)作为一种高发的退行性关节疾病,其特征为慢性疼痛和活动能力丧失,极大影响中老年人群的生活质量。当前治疗手段以缓解症状为主,缺乏延缓疾病进展的创新药物,临床需求亟待满足。2025年8月14日,国家药品监督管理局药品审评中心发布《骨关节炎新药临床研发技术指导原则》,旨在规范临床研究设计,优化疗效评估体系,并推动靶向治疗与精准医学的探索。本文系统解读该指导原则的核心内容,重点分析探索性与确证性临床研究的关键问题,探讨骨关节炎药物研发挑战与未来临床研发方向,以期为骨关节炎新药研发提供科学参考。

Abstract

Osteoarthritis (OA), a highly prevalent degenerative joint disease, is characterized by chronic pain and loss of mobility, which significantly affects the quality of life of middle-aged and elderly people. Current treatments methods mainly focus on symptom relief, and there is a lack of innovative drugs that can delay disease progression. To address these unmet clinical needs, the Center for Drug Evaluation of the National Medical Products Administration issued the “Guidance for Clinical Research and Development of New Drugs for Osteoarthritis” on August 14, 2025. The guidance aims to standardize clinical study design, optimize the efficacy evaluation system, and promote exploration of targeted therapy and precision medicine. This article systematically interprets the core content of this guidance, with a focus on key issues in exploratory and confirmatory clinical studies, and also discusses challenges in drug development for osteoarthritis and future directions for clinical research, aiming to provide a scientific reference for new drug development in this field.
