基于TSP的情景模拟教学法在急诊住院医师规范化培训中的应用
Application of the TSP-based scenario simulation teaching method in standardized training for emergency resident physicians
目的 探讨基于教师标准化病人(teachers standardized patient,TSP)的情景模拟教学法在急诊住院医师规范化培训中的应用。 方法 选取在滨州医学院附属医院急诊基地进行规范化培训的住院医师60名,随机分为对照组(n=30)和试验组(n=30)。试验组采用TSP联合情景模拟教学法,对照组采用传统教学方法。培训结束后,采用客观结构化临床考试(OSCE)、Mini-CEX量表和教学满意度评价比较两组教学效果差异。 结果 试验组的OSCE成绩总分高于对照组的总分(81.2±6.23 vs.72.8±5.59,P<0.001);试验组的Mini-CEX整体表现评分高于对照组的评分(7.67±0.88 vs.5.53±0.73,P<0.001)。教学满意度评价,对照组不满意、一般、满意和非常满意的比例分别为0.0%、26.7%、56.7%、16.7%,试验组的比例分别为0.0%、6.7%、53.3%、40.0%,差异有统计学意义(P=0.012)。 结论 基于TSP的情景模拟教学法在急诊科住院医师规范化培训中取得了较好的效果,值得进一步推广。
Objective To explore the application of the teachers standardized patient (TSP)-based scenario simulation method in standardized training for emergency resident physicians. Methods A total of 60 resident physicians undergoing standardized training at the Emergency Training Base of Binzhou Medical University Hospital were randomly divided into control group and experimental group. The experimental group (30 participants) was taught with TSP combined with the scenario simulation teaching method, while the control group (30 participants) was tanght with traditional teaching methods. After completion of the training, the objective structured clinical examination (OSCE), Mini-CEX scale, and teaching satisfaction evaluation were used to compare the teaching effectiveness in the two groups. Results The overall OSCE scores were higher in the experimental group than in the control group (81.2±6.23 vs. 72.8±5.59, P<0.001); the overall performance of the Mini-CEX scores were also significantly higher in the experimental group (7.67±0.88 vs. 5.53±0.73, P<0.001). In terms of teaching satisfaction evaluation, the proportions of dissatisfaction, average, satisfaction and high satisfaction in the control group were 0.0%, 26.7%, 56.7% and 16.7%, respectively, while in the experimental group they were 0.0%, 6.7%, 53.3% and 40.0%, respectively, with a statistically significant difference (P=0.012). Conclusion The TSP-based scenario simulation teaching method demonstrates better effects than traditional teaching methods in emergency medicine residency education and thus should be further promoted.
