基于数字化技术的口腔临床实习前强化培训体系构建及应用
Construction and application of a pre-clinical intensive training system for dental practice based on digital technology
目的 构建基于数字化技术的口腔临床实习前强化培训体系,探讨其在提高学生临床实践能力和临床思辨能力方面的应用价值。 方法 以某医学院口腔医学专业本科生为研究对象,构建包含口腔基本检查、临床前模拟技能训练和临床思辨能力训练三个模块的强化培训体系。其中,临床前模拟技能训练采用过程评估系统、结果评估系统及虚拟仿真牙医培训机进行数字化训练;临床思辨能力训练采用口腔人工智能机器人和标准化病人进行混合式训练。通过即时评价和追踪评价评估培训效果。 结果 过程评估和结果评估系统显示:学生G.V.Black Ⅱ类洞制备训练平均操作分值显著提高(P<0.01),根管预备训练中操作准确度、预备深度控制及轴向控制能力均显著提升(P<0.05);磨牙铸造金属全冠预备的边缘完整性与轴面预备均匀度的评分及合格率均显著增长(P<0.01),整体完成质量合格率提高约20%;特殊人群问诊能力提升13.63%(P=0.032);临床焦虑评分显著降低[从(6.82±1.24)分降至(4.73±0.92)分,P=0.006];85.00%的带教教师认为学生临床综合能力有明显提升。 结论 基于数字化技术的口腔临床实习前强化培训体系对提高学生的临床实践能力和临床思辨能力具有积极作用,为口腔医学教育改革提供了新的思路。
Objective To construct a pre-clinical intensive training system based on digital technology and explore its value in improving students’ clinical practice ability and clinical reasoning ability. Methods Taking undergraduate students majoring in Stomatology of a medical college as the research objects, a training system of three modules including basic oral examination, pre-clinical simulation skill training, and clinical reasoning ability training was constructed. The pre-clinical simulation skill training conducted digital training through process evaluation system, result evaluation system and virtual dental training machine; clinical reasoning ability training applied blended training using dental artificial intelligence robot and standardized patient (SP). The training effect was evaluated through immediate and follow-up assessments. Results The process assessment system showed that students’ average operational scores in G.V.Black Class II cavity preparation increased significantly (P<0.01), and the accuracy, depth control, and axial control in root canal preparation all improved significantly (all P<0.05). Significant improvements in both scores and pass rates for marginal integrity and axial preparation uniformity in molar cast metal crown preparation (all P<0.01), with an approximately 20% increase in the overall quality pass rate. Special population inquiry ability increased by 13.63% (P=0.032). Clinical anxiety score significantly decreased from (6.82±1.24) to (4.73±0.92) (P=0.006). 85.00% of clinical teachers reported notable improvement in students’ comprehensive clinical ability. Conclusion The pre-clinical intensive training system based on digital technology has a positive effect on improving students’ clinical practice ability and clinical reasoning ability, providing new insights for dental education reform.
