大语言模型引导下PBL联合iSIM在骨科临床实习教学中的应用

大语言模型引导下PBL联合iSIM在骨科临床实习教学中的应用
Application of PBL combined with iSIM in clinical orthopedic internship teaching under the guidance of large language model
目的 探索大语言模型引导下以问题为中心的教学法(problem-based learning, PBL)联合情景模拟教学(improving simulation instructional methods, iSIM)在骨科临床实习教学中的应用效果。 方法 以滨州医学院烟台附属医院骨科100 名本科实习生作为研究对象,将学生随机分为试验组和对照组(每组50人)。试验组采用大语言模型引导下的PBL联合iSIM教学模式,对照组采用传统教学法(lecture based learning, LBL)教学模式。通过教学综合成绩、学生对教学方法评价、带教教师对教学方法评价三方面进行教学效果综合评价。 结果 大语言模型引导下PBL联合iSIM的试验组教学综合成绩为(90.5±5.1)分,优于对照组的教学综合成绩(80.8±7.3)分(t=7.671,P<0.001);学生对教学方法的评价方面,试验组的评价优于对照组(两组评价为“十分满意”“满意”的比例分别为66.0%、20.0%和10.0%、10.0%,Z=6.407,P<0.001);带教教师对教学方法的评价方面,试验组的评价优于对照组(两组评价为“十分满意”“满意”的比例分别为58.0%、16.0%和10.0%、30.0%,Z=4.919,P<0.001)。 结论 大语言模型引导下PBL联合iSIM教学模式可有效提高骨科临床实习生学习效果,值得进一步推广。
Objective To explore the application effect of PBL combined with iSIM under the guidance of large language model in clinical orthopaedic internship teaching. Methods 100 undergraduate interns rotating in the orthopaedic department of Yantai Affiliated Hospital of Binzhou Medical University were selected as the study subjects, and divided into the experimental group and the control group respectively, with 50 students in each group. The experimental group adopted the new teaching mode of PBL combined with iSIM under the guidance of large language model, and the control group adopted the traditional teaching mode of LBL. The teaching effect was evaluated through the three aspects of the comprehensive teaching achievement, students’ satisfaction with the teaching method, and the satisfaction of the teaching teachers with the teaching method. Results The overall score of the experimental group taught by PBL combined with iSIM under the large language model was 90.5±5.1 points, and that of the control group was (80.8±7.3) points (t=7.671, P<0.001); Regarding students’ evaluations of teaching methods, the experimental group outperformed the control group (the proportions of students rating ‘highly satisfied’ and ‘satisfied’ were 66.0% and 20.0% versus 10.0% and 10.0% respectively; Z=6.407, P<0.001); In terms of teaching staff evaluations of teaching methods, the experimental group outperformed the control group (the proportions of ‘highly satisfied’ and ‘satisfied’ responses were 58.0% and 16.0% versus 10.0% and 30.0% respectively, Z=4.919, P<0.001). Conclusion The PBL combined with iSIM teaching mode guided by the large language model improves the learning effect of clinical interns rotating in orthopaedics significantly and is worth promoting.
