摘要

目的 探讨不同影响因素对肺结节患者预后的影响,为临床诊疗提供科学依据。方法 从中国知网、中国生物医学文献服务系统、PubMed、Cochrane Library、Embase、Web of Science数据库中收集肺结节患者预后不同影响因素的队列研究和病例对照研究,检索时间为2014年3月1日至2024年2月29日。研究采用Revman 5.3 和 Stata 18 进行,提取相关数据,包括 WMDOR、95%CI等。结果 共纳入 13 个研究,研究对象 4011 例。Meta分析结果显示,年龄(WMD=-5.35,P<0.001,I2=82%)、家族史(OR=0.03,P<0.001,I2=97%)、吸烟史(OR=0.18,P<0.001,I2=95%)、结节位置(OR=1.98,P<0.001,I2=87%)、结节直径(WMD=-2.13,P<0.001,I2=42%)、分叶征(OR=1.53,P=0.003,I2=97%)、空洞征(OR=33.71,P<0.001,I2=99%)、毛刺征(OR=2.32,P<0.001,I2=95%)、空泡征(OR=0.54,P<0.001,I2=99%)、神经元特异性烯醇化酶(WMD=-1.25,P<0.001,I2=74%)是肺结节患者预后不良的影响因素(P<0.05)。结论 肺结节患者预后情况与年龄大、有家族史、既往有吸烟史、结节位于上叶、直径大、出现分叶征、空洞征、毛刺征、空泡征、神经元特异性烯醇化酶质量浓度升高多种因素相关。

Abstract

Objective To investigate the effects of different influencing factors on the prognosis of patients with pulmonary nodules,and to provide a scientific basis for clinical diagnosis and treatment.Methods Cohort studies and case-control studies with different influencing factors of prognosis of patients with pulmonary nodules were collected from China National Knowledge Infrastructure,China Biomedical Literature Service System,PubMed,Cochrane Library,Embase,and Web of Science databases,and the search period was from March 1,2024 to February 29,2024.The study was conducted using Revman5.3 and Stata18,and relevant data were extracted,including WMDOR,95%CIResults A total of 13 studies were conducted,involving 4011 study subjects.The results of Meta-analysis showed that age(WMD=-5.35,P<0.001,I2=82%),family history (OR=0.03,P<0.001,I2=97%),smoking history (OR=0.18,P<0.001,I2=95%),nodule location (OR=1.98,P<0.001,I2=87%),nodule diameter(WMD=-2.13,P<0.001,I2=42%),lobulation signs(OR=1.53,P=0.003,I2=97%),cavitation signs(OR=33.71,P<0.001,I2=99%),spiculation sign(OR=2.32,P<0.001,I2=95%),vacuole signs(OR=0.54,P<0.001,I2=99%),neuron-specific enolase(WMD=-1.25,P<0.001,I2=74%)were the influencing factors for poor prognosis in patients with pulmonary nodules(P<0.05). Conclusion The prognosis of patients with pulmonary nodules is associated with ad-vanced age,family histo-ry,smoking history,nodules located in the upper lobes,large nodule diameter,and the presence of lobulation,cavitation,spiculation or vacuole signs,as well as elevated neuron-specific enolase.
