摘要

目的 探讨Phoenix评分在儿童脓毒症的应用价值及预后影响因素。 方法 回顾性收集2018年1月—2021年12月首都医科大学附属北京儿童医院保定医院儿童重症监护病房确诊为严重脓毒症及脓毒性休克的患儿96例。收集患儿临床资料,包括性别、年龄、基础病、病原资料、器官功能障碍、血清胆红素值、格拉斯哥昏迷评分(Glasgow coma scale,GCS)、血清肌酐等数值计算儿童序贯器官衰竭评估(pediatric sequential organ failure assessment,pSOFA)评分、第三代小儿死亡危险(pediatric risk of mortality Ⅲ,PRISMⅢ)评分,并统计血清乳酸、C反应蛋白、降钙素原、纤维蛋白原(fibrinogen,FIB)、国际标准化比值 (international normalized ratio,INR)、D-二聚体、血小板等数值。根据预后不同将入选患儿分为了存活组和预后不良组,采用Logistic回归分析确定脓毒症患儿预后的影响因素。 结果 以2005年国际专家共识标准诊断为严重脓毒症的患儿共96例,其中脓毒性休克患儿46例。以Phoenix评分标准可诊断为脓毒症的患儿共91例,阳性率为94.8%,排除者5例,阴性率5.2%。Phoenix评分标准可诊断为脓毒症休克的患儿70例,与原有脓毒症休克患儿存在一定交叉。脓毒症患儿91例,根据临床转归情况分为存活组51例和预后不良组40例。预后不良组FIB低于存活组,D-二聚体、GCS评分、乳酸、血管活性药数量、确诊时Phoenix评分、确诊时pSOFA评分以及确诊时PRISMⅢ评分高于存活组(P<0.05)。Logistic回归分析结果表明,GCS和FIB水平为严重脓毒症患儿预后转归的影响因素(OR=3.736、2.897,P=0.010、0.039),FIB数值越低,昏迷程度越深,预后越差。脓毒性休克患儿70例,根据临床转归情况分为存活组38例和预后不良组32例。预后不良组FIB低于存活组,INR、D-二聚体、GCS评分、乳酸、血管活性药数量、确诊时Phoenix评分、确诊时pSOFA评分以及确诊时PRISMⅢ评分高于存活组(P<0.05);Logistic回归分析结果显示,GCS评分是脓毒性休克患儿预后转归的影响因素(OR=3.841,P=0.021),GCS评分越低,昏迷程度越深,预后越差。 结论 Phoenix评分标准在儿童脓毒症的识别方面具有较高的临床应用价值,有助于更早启动干预。FIB和GCS评分是脓毒症患儿不良预后的影响因素。FIB水平降低可作为脓毒症诱发弥散性血管内凝血的早期预警指标,为临床早期干预提供了参考依据。GCS评分作为评估神经系统功能的重要指标,其下降提示脑功能障碍,这在脓毒症患儿中常预示着更高的死亡风险。

Abstract

Objective To investigate the application value of the Phoenix scoring system in pediatric sepsis and analysis of prognostic factors. Methods We retrospectively collected data of 96 children diagnosed with severe sepsis and septic shock in the Pediatric Intensive Care Unit (PICU), Baoding Hospital of Beijing Children's Hospital, Capital Medical University from January 2018 to December 2021. Clinical data of the pediatric patients, including gender, age, underlying diseases, pathogen information, organ dysfunction, serum bilirubin levels, Glasgow Coma Scale (GCS) score, serum creatinine, and other values were collected to calculate the Pediatric Sequential Organ Failure Assessment (pSOFA) score and the Pediatric Risk of Mortality Ⅲ (PRISM Ⅲ) score. Additionally, serum lactic acid, C-reactive protein(CRP), procalcitonin(PCT), fibrinogen (FIB), international normalized ratio (INR), D-dimer (DD), platelet count, and other relevant values were recorded.Based on different prognoses, the enrolled children were divided into a survival group and a poor prognosis group. Logistic regression analysis was used to determine the influencing factors of prognosis in children with sepsis. Results A total of 96 children were diagnosed with severe sepsis according to the 2005 International Consensus Criteria, including 46 patients with septic shock. Based on the Phoenix criteria, 91 children were diagnosed with sepsis, yielding a positive rate of 94.8%, while 5 children were excluded, with a negative rate of 5.2%. The Phoenix criteria identified 70 patients with septic shock, showing some overlap with the original septic shock patients. Among the 91 sepsis patients, they were divided into a survival subgroup (n=51) and a poor prognosis subgroup (n=40) based on clinical outcomes. The poor prognosis subgroup had lower FIB levels than the survival subgroup, while DD, GCS score, lactic acid, number of vasoactive drugs, Phoenix score at diagnosis, pSOFA score at diagnosis, and PRISM Ⅲ score at diagnosis were higher in the poor prognosis subgroup (P<0.05). Logistic regression analysis revealed that GCS and FIB levels were influencing factors for the prognosis of severe sepsis (OR=3.736, 2.897, P=0.010, 0.039). Lower FIB values correlated with deeper coma and worse prognosis. Among the 70 septic shock patients, they were divided into a survival subgroup (n=38) and a poor prognosis subgroup (n=32) based on clinical outcomes. The poor prognosis subgroup had lower FIB levels than the survival subgroup, while INR, DD, GCS score, lactic acid, number of vasoactive drugs, Phoenix score at diagnosis, pSOFA score at diagnosis, and PRISM Ⅲ score at diagnosis were higher in the poor prognosis subgroup (P<0.05). Logistic regression analysis showed that the GCS score was an influencing factor for the prognosis of septic shock (OR=3.841, P=0.021), with lower GCS scores indicating deeper coma and worse prognosis. Conclusion The Phoenix scoring criteria demonstrate high clinical application value in the identification of pediatric sepsis, facilitating earlier intervention initiation. Both FIB and GCS scores are influencing factors for poor prognosis in children with sepsis. Decreased FIB levels can serve as an early warning indicator for sepsis-induced disseminated intravascular coagulation, providing a reference basis for early clinical intervention. As a crucial metric for assessing neurological function, a decline in GCS score indicates cerebral dysfunction, which often predicts higher mortality risk in pediatric sepsis patients.

脓毒症对儿童健康的威胁不容忽视,是各年龄段患者死亡和残疾的主要原因。2017年,全球与脓毒症相关的死亡例数占当年死亡例数的19.7%1。2023年中国最新脓毒症流行病学研究显示,2017―2019年共有13.1%为脓毒症相关死亡,凸显了儿童脓毒症早期识别和有效干预的紧迫性2。2017年全球所有脓毒症患者中有一半以上发生在儿童中,其中许多是新生儿1。儿童脓毒症作为一种严重的感染性疾病,始终是儿科领域研究的重点方向。随着医学研究的深入,其定义和诊断标准也在持续更新和完善。2005年,国际儿科领域专家达成共识3,将脓毒症界定为全身炎症反应综合征(systemic inflammatory response syndrome,SIRS)与感染并存的状态。其中,当脓毒症患者出现心血管系统功能异常或急性呼吸窘迫综合征,或同时存在2个及以上其他器官系统功能障碍时,则被归类为严重脓毒症。若脓毒症患者伴随心血管系统功能障碍,则被诊断为脓毒性休克。直至2024年,经收集国际、多中心的数据,制定更新了Phoenix评分标准4。Phoenix评分系统的引入为早期诊断与治疗提供了有力支持。该系统通过综合分析患儿生命体征、实验室指标等多维度数据,能够精准评估病情严重程度,指导临床及时干预,有效提升了救治成功率。本研究采用回顾性数据分析方法,旨在评估Phoenix评分系统在儿童脓毒症诊断中的应用价值,并深入探讨重症脓毒症患儿临床转归的影响因素,以期为改善临床预后提供依据。现报告如下。

1 资料与方法

1.1 一般资料

选取2018年1月1日—2021年12月31日于首都医科大学附属北京儿童医院保定医院儿童重症监护病房(pediatric intensive care unit,PICU)收治的符合严重脓毒症及脓毒性休克诊断标准的患儿96例。纳入标准:①于PICU住院治疗;②年龄29 d~18周岁;③符合严重脓毒症或脓毒性休克诊断标准的患儿。诊断依据参照2005年发布的《国际儿童脓毒症共识会议:脓毒症及相关器官功能障碍的定义》3执行。排除标准:①诊断48 h内未采集病原学检测者;②资料不完整者。

本研究经首都医科大学附属北京儿童医院保定医院医学伦理委员会批准通过(批准文号:2025(年)快审【科】第(13)号)。

1.2 方法

1.2.1 分组

根据患儿预后转归,分为存活组及预后不良组,其中预后不良组包括死亡患儿、放弃治疗患儿及病情进展转上级医院患儿。

1.2.2 资料收集及观察指标

包括性别、年龄、基础病、机械通气、器官功能障碍、格拉斯哥昏迷评分(Glasgow coma scale,GCS)、血管活性药物数量,收集距离确诊时间最近的血清乳酸、C反应蛋白(C-reactive protein,CRP)、降钙素原(procalcitonin,PCT)、纤维蛋白原(fibrinogen,FIB)、国际标准化比值(international normalized ratio,INR)、D-二聚体、血小板、血清胆红素值、血清肌酐等数值,并计算每例患儿的儿童序贯器官衰竭评估(pediatric sequential organ failure assessment,pSOFA)评分、第三代小儿死亡危险(pediatric risk of mortality Ⅲ,PRISMⅢ)评分。

1.3 统计学方法

应用SPSS 25.0统计软件分析数据。计量数据比较采用独立样本t检验,计数资料比较采用χ2检验,采用Logistic回归模型确定脓毒症患儿预后的的影响因素。P<0.05为差异有统计学意义。

2 结 果

2.1 一般资料

本研究共纳入符合2005年国际专家共识诊断标准的严重脓毒症患儿96例,年龄2个月~14岁,平均(4.57±4.10)岁,其中包含脓毒性休克患儿46例。所有患儿均参照《儿童脓毒性休克(感染性休克)诊治专家共识(2015版)》5进行诊疗,严格执行3 h集束化治疗方案,大部分病例可完成6 h集束化治疗。经Phoenix脓毒症评分系统评估,符合脓毒症诊断标准的患儿91例,诊断阳性率为94.8%,排除5例,阴性率为5.2%(图1)。在脓毒性休克诊断方面,该评分系统识别出阳性病例70例,与临床诊断结果存在部分重叠(2005版国际共识标准诊断脓毒性休克为46例,其中44%可应用Phoenix评分标准同时诊断为脓毒性休克;2005版国际共识标准为非休克的严重脓毒症中,可用Phoenix评分标准诊断为脓毒性休克者占总数的29%(图2)。根据临床转归情况,将研究对象划分为存活组和预后不良组,其中预后不良组包含死亡病例、自动出院病例以及因病情恶化转至上级医疗机构的病例。

2.2 脓毒症预后的影响因素分析

脓毒症患儿91例,根据临床转归情况分为存活组51例和预后不良组40例。预后不良组FIB低于存活组,D-二聚体、GCS评分、乳酸、血管活性药数量、确诊时Phoenix评分、确诊时pSOFA评分以及确诊时PRISMⅢ评分高于存活组,差异有统计学意义(P<0.05)。2组性别、年龄、基础病、机械通气,血小板、CRP、PCT、INR差异无统计学意义(P>0.05)。见表1。以患儿临床转归作为因变量,FIB、D-二聚体、GCS、血乳酸水平、血管活性药数量、确诊时Phoenix评分、确诊时pSOFA评分以及确诊时PRISMⅢ评分为自变量构建Logistic回归模型,变量赋值见表2。结果表明,GCS和FIB水平为严重脓毒症患儿预后转归的影响因素(P<0.05),见表3。GCS越低,昏迷程度越深,FIB数值越低,提示预后不良。

2.3 脓毒性休克预后的影响因素分析

3 讨 论

脓毒症是全球范围内导致儿童死亡和残疾的重要原因。儿童脓毒症与成人脓毒症存在重要差异,包括生命体征的年龄特异性变异、年龄依赖性、免疫功能的发育、儿科特异性病原体、合并症、流行病学和结局的差异。儿童脓毒症的诊断标准体系始于2005年首次提出的定义框架3,该框架首次明确了儿童脓毒症、严重脓毒症及脓毒性休克的分级诊断标准,并基于儿童年龄特征和发育特点对相关生理参数进行了适应性调整。本研究回顾性分析了96例符合2005年国际专家共识标准诊断的严重脓毒症患儿。随着成人Sepsis 3.0标准6的发布,2016年学界开始探索将其核心概念应用于儿科领域,由此衍生出pSOFA7。2020年,临床实践指南更新聚焦于儿童脓毒性休克的诊疗规范8。儿童脓毒症的核心诊断标准已近二十年未作实质性修订,直到2024年美国重症医学会研究小组收集了孟加拉国、中国、哥伦比亚和肯尼亚的6个经济较发达地区和4个经济较落后地区的卫生系统数据才推出了Phoenix标准4

2005年国际专家共识标准与Phoenix评分比较,2005年国际专家共识标准将脓毒症定义为SIRS与感染并存的状态,并进一步细分为严重脓毒症(伴心血管系统功能异常或急性呼吸窘迫综合征,或同时存在2个及以上其他器官系统功能障碍)和脓毒性休克(伴心血管系统功能障碍)3。这种基于SIRS的定义在临床实践中存在一定的局限性,例如SIRS标准敏感度高但特异度不足,可能导致过度诊断,且未能充分反映器官功能障碍的严重程度9-10。相比之下,2024年发布的Phoenix评分标准对儿童脓毒症的定义进行了根本性变革,不再应用基于SIRS的标准,且术语“严重脓毒症”不再继续使用,因为脓毒症本身即被定义为与感染相关的危及生命的器官功能障碍。新的脓毒症Phoenix评分涉及心血管、呼吸、神经和凝血功能障碍4个系统,疑似或确诊感染的儿童,Phoenix评分≥2分诊断脓毒症,提示可能出现致命的器官功能障碍;其中,Phoenix评分的循环系统得分≥1分即可诊断脓毒性休克4

本研究结果显示,Phoenix评分标准诊断儿童脓毒症的阳性率为94.8%,阴性率为5.2%,提示其在儿童脓毒症的识别方面具有较高的临床应用价值。在诊断脓毒性休克方面,本研究中Phoenix评分诊断出脓毒性休克70例,而2005年国际专家共识标准诊断脓毒性休克46例。这表明Phoenix评分可能识别出更多处于脓毒性休克状态的患儿,有助于更早启动干预。具体而言,2005年标准诊断为脓毒性休克的病例中,有44%可被Phoenix评分标准同时诊断为脓毒性休克;而在2005年标准诊断为非休克的严重脓毒症病例中,有29%可被Phoenix评分标准诊断为脓毒性休克。这一差异可能与Phoenix评分中应用血管活性药物1种及以上即可使循环系统得分≥1分的特点有关,尽管临床工作中血管活性药物的应用存在一定主观性,但其更强调早期循环功能障碍的识别4。多项国内外研究也支持Phoenix评分在诊断准确性方面的优势。Sanchez-Pinto等11在一项美国九中心数据集的回顾性验证研究中发现,Phoenix标准在区分感染危重症患儿病死率方面优于2005年国际专家共识标准,并在所有亚组分析(年龄、恶性肿瘤、技术依赖性)中均表现出更高的性能。该研究指出,43%的患儿符合Phoenix脓毒症或脓毒性休克标准,其中院内病死率为9%。值得注意的是,28%符合Phoenix标准的患者不符合2005年国际专家共识标准,但其院内病死率(4.7%)高于仅符合2005年国际专家共识标准但不符合Phoenix标准的患者(1.7%),这进一步突出了Phoenix标准识别高危患者的能力11。Phoenix标准的开发和验证过程也体现了其先进性,其利用了来自美国、哥伦比亚、孟加拉国、中国和肯尼亚等10个卫生系统超过360万儿科就诊数据,涵盖了不同资源水平的地区,从而增强了其普适性4。然而,也有研究指出Phoenix评分在特定人群中的局限性。一项欧洲多中心回顾性研究评估了Phoenix评分标准在脓毒症儿科肿瘤患者中的预后准确性,发现其对90 d全因病死率的预测准确性较差(AUC为0.66),对PICU病死率的预测性能也低于可接受水平12。这可能因为儿科肿瘤的器官功能障碍不仅归因于感染,还可能受到癌症本身及其治疗的影响,从而影响病死率预测12。这提示在应用Phoenix评分时,需要考虑患儿的特殊临床背景。

本研究首次将Phoenix评分标准应用于我国儿童脓毒症及脓毒性休克的临床诊断和预后评估,为该评分体系在儿科领域的应用提供了初步研究依据。通过对比2种标准在诊断病例数量上的差异,本研究提示Phoenix评分可能识别出更多处于脓毒性休克状态的患儿,有助于更早启动干预。

本研究进一步探讨了FIB和GCS评分在儿童脓毒症预后评估中的作用。分析影响脓毒症患儿预后的诸多因素,发现FIB和GCS评分是脓毒症患儿不良预后的影响因素,FIB数值越低,昏迷程度越深,提示预后越差。Wang等13、朱伟奇等14、白欢等15研究得出,D-二聚体、FIB、INR是脓毒症患儿预后的影响因素。凝血功能紊乱在脓毒症病理过程中扮演着重要角色,是引发微循环障碍和多器官功能衰竭的核心病理环节16-17。脓毒症状态下,机体凝血系统呈现过度激活状态:组织因子介导凝血酶生成,促使血小板活化并加速纤维蛋白原向纤维蛋白转化;同时,生理性抗凝机制受损,多种凝血途径受到抑制,加之纤溶酶原激活物抑制物水平升高,导致纤维蛋白微血栓清除障碍,这些因素共同构成了脓毒症病情恶化的重要病理基础18-19。FIB作为一种肝源性糖蛋白,在脓毒症病理过程中具有双重作用20-24:其一,在凝血酶作用下生成纤维蛋白单体,参与止血过程;其二,通过与血小板糖蛋白相互作用,介导血小板聚集,影响血液流变学特性,促进血栓形成。值得注意的是,纤维蛋白沉积还可诱导白细胞向炎症部位趋化,加剧局部炎症反应25-30。Napoleone等31研究证实,脓毒症早期即可通过外源性途径激活凝血系统。在脂多糖和炎症介质刺激下,血管内皮细胞和单核细胞表达组织因子,与活化因子Ⅶ形成复合物,同时释放肿瘤坏死因子和白细胞介素1。在Ca2+参与下,因子Ⅹ被激活。此外,炎症因子还可诱导内皮细胞表达多种促凝物质,包括凝血调节蛋白、凝血酶受体、血管性血友病因子等,以及E-选择素、细胞间黏附分子1、血管细胞黏附分子1等黏附分子,促进白细胞-内皮细胞间的相互作用,进一步放大炎症反应32-35。脓毒症引起的凝血病范围从轻微的凝血激活到更严重的弥散性血管内凝血,其特征是同时广泛的微血管血栓形成和各部位大量出血36-38。Su等39通过临床观察发现,脓毒症患者病程进展过程中,凝血酶原时间、FIB、D-二聚体及INR呈现进行性升高趋势,提示机体存在高凝状态伴继发性纤溶亢进,且凝血与纤溶系统之间存在显著相关性。研究进一步证实,死亡病例组上述指标水平显著高于存活组,表明继发性纤溶系统的激活在脓毒症病理生理过程中发挥关键作用,其亢进程度与疾病严重程度呈正相关。项龙等40研究显示,脓毒症病程中凝血功能紊乱的动态演变特征:早期以高凝状态为主,随病情进展逐渐转变为低凝状态。Fang等41研究指出,活化部分凝血活酶时间延长和FIB水平降低可作为脓毒症诱发弥散性血管内凝血的早期预警指标,为临床早期干预提供了参考依据。

GCS评分作为评估神经系统功能的重要指标,其下降提示脑功能障碍,这在脓毒症患儿中常预示着更高的死亡风险。Meral等42的一项前瞻性观察队列研究中,死亡组GCS评分明显低于非死亡组,GCS评分可作为危重患者独立实用的病死率预测模型。一个多中心研究43发现,在所研究的401例儿童中,71例(18%)出现了急性意识障碍,以急性意识障碍为主要表现的患儿群体呈现以下特征:年龄分布显著偏大(中位年龄:8岁 vs. 5岁,P=0.023);住院期间病死率明显升高(21% vs. 10%,P=0.011);多器官功能衰竭发生率显著增加(93% vs. 71%,P<0.001);同时,巨噬细胞活化综合征的发生频率也明显高于对照组(14% vs. 4%,P=0.004)。Hu等44采用SHapley Additive exPlanations(SHAP)方法构建预后预测模型,通过量化特征贡献度评估各预测变量的影响权重,结果表明,GCS在脓毒症患者死亡风险预测中具有显著贡献度,被确定为最重要的独立预测因子。将这些指标纳入Phoenix评分体系的综合评估,有助于临床医生更全面地判断患儿病情严重程度和预后风险,从而指导个体化治疗策略的制定。

尽管本研究为Phoenix评分在儿童脓毒症中的应用提供了初步证据,但仍存在一定的局限性。首先,本研究为单中心回顾性研究,样本量相对有限(n=96),可能导致选择偏倚,并限制了研究结果的普适性;其次,近年来脓毒症诊疗方案不断更新,本研究纳入的历史病例数据可能存在时效性偏倚,影响结论在当前临床环境中的适用性;再次,数据收集方面存在局限性,例如52例气管插管患者缺乏详细的插管时机和镇静药物使用情况等关键信息,这可能影响GCS评分的准确评估。尽管采用了分级记录方法(>10分、4~10分、≤3分)对GCS评分进行标准化处理,但这种处理方式仍可能引入一定的测量偏倚。 鉴于上述局限性,未来的研究应开展多中心、前瞻性、大样本队列研究,以进一步验证Phoenix评分系统在不同人群和不同医疗环境下的诊断和预后价值。例如,一项在韩国高资源PICU进行的研究验证了 在预测感染PICU患者病死率方面的可靠性,并指出其AUPRC显著高于pSOFA评分,表明其在病死率预测方面具有优越性能45。另一项在英国急诊转运PICU队列中的探索性分析也发现,较高的Phoenix评分与更高的病死率相关,并认为Phoenix评分标准可用于急诊PICU人群的风险分层46。此外,可以结合其他新型生物标志物,如降钙素原、CRP、乳酸等,以及影像学检查,构建更完善的儿童脓毒症早期预警和风险评估模型47。深入研究Phoenix评分指导下的早期干预措施对儿童脓毒症预后的影响,以及探索其在资源有限地区的适用性,将对改善全球儿童脓毒症的临床管理和预后具有重要意义。
